demonstrated willingness to live and work in a regional centre outside of Darwin."

I think that means that when they assess your sponsorship application they may approve it but on the condition that you live and work outside of Darwin, or they may not put this condition on and leave you free to work in Darwin. Whether or not they impose this condition will be dependent upon the exact skills shortage in different areas for your profession.
