Seemingly they are still used in some places where phone or internet services are not available although how they work with modern printed cards with no raised characters I don't know, probably have to hand write the numbers on the vouchers.
Re 'cash only' establishments. I have no problem with that so long as they make it crystal clear from the the moment you set foot in the door.
An Indian restaurant not far from me switched to a cash only policy but decided to appraise us of the change only after we'd eaten and were presented with the bill. We didn't have sufficient cash on us and there were no ATM's nearby (not sure I'd have put up with the inconvenience of going and coming back anyway, or the charges for using a 3rd party machine) so we ended up getting about 25% off, a hell of a bigger loss to the restaurant than the card fees would have been!
They have since displayed their policy rather more prominently.
