The last I read was continued funding by UK for Brits in EU with S1, was not guaranteed indefinitely. It would be conditional on a reciprocal agreement with EU.

The right to UK-funded state healthcare in the EU country in which a UK citizen is resident on 31st December 2020 is included in the withdrawal agreement, which has been ratified and is in force. What is not certain is healthcare need of those moving to EU from 1st January 2021, who are not beneficiaries of the withdrawal agreement. As things stand, UK is no longer funding such people, except for travellers and other short-term visitors who can continue to use EHIC, or the new GHIC. Currently there is no agreement by UK and EU over the healthcare need of residents from the start of 2021, though it's possible that both parties can reopen negotiation. During 2020, some people expressed doubt over their continued access to state healthcare from 2021, but their fears have been laid to rest through government statements.

I sincerely hope you are correct. I little confidence in any assurances given by UK plc.
I can confirm that I have received free health care from hospitals in Spain. Others have not been so fortunate. Even before Brexshit, there were reports from tourists who were asked to pay despite having valid EHIC Card.

There have always been (hopefully isolated) cases of people unable to exercise their rights under EU rules or any negotiated settlement. Over the years, I have received free treatment by producing EHIC or previous E111 in the Netherlands, Germany and Italy. When it comes to registering for healthcare in Spain, I would hope that TGSS and Centro de Salud offices will know what they are doing.

It seems I started my retirement plan a year too late All my enquiries seem to suggest I am going to have to limit myself to 90 days.
The problem I am having is spending 6 months in each country and getting medical cover for 6 months whilst abroad.
Does anybody else have suggestions as it looks like S1 is out of the question
Can you join Spanish health care for 6 months as all enquiries so far have suggested a year is only option

You can't join Spanish national healthcare as you need to have been resident here for a year. Then there is private healthcare, either pay-as-you-go (lots of private doctors in Spain) or through a UK policy. If you are UK resident, you can still use EHIC for up to 3 months, or the new GHIC, both of which is issued free and gives you free or subsidised public healthcare on the same term as locals.

Plan N as plan B seems a long time ago
looks like I am going to have to go down the route of 90 days in 180 for this year using Annual travel insurance.
Been given an option of 6 months cover in Spain so will now look at what Visa I need to get for staying from Oct 14th 2021 to Mar 30th 2022

There is no visa for this you are either a tourist maximum 90 days or, if you qualify, a resident with a non-lucrative visa.

End of story.

Thanks for everyone’s input

You will need one of the residence visas, there is no other option. Remembering that it will have to be applied for in UK before travelling.
