Thanks Snikpoh, I think that's what I'll do. Will have to get onto Google translate now 'cos form is in Spanish so don't know what questions are on it!

It is absolutely not your place to be filling in an E121, it is (was) an official document issued by DWP, but if that is what they are asking you to do then what the hell !
Don't you just love the way some of these government agencies come up with their own completely off the wall rules and requirements, it keeps us on our toes.
