I had a root canal, the wife had a crown and I had a filling as well for €400
Fillings generally €40 here, last one I had done in the uk was £150!!!

Really?
I thought the law stated that you had to register as a resident if you lived here.

Assuming Las Chapas is in the Mijas Costa district (you need to check as it may come under Marbella)...

To register at the Doctor (in La Cala de Mijas) you need to go to the Town Hall (just round the corner from the Doctor) and register which is an easy process and everyone should do that anyway. Then armed with your thingy from the Town Hall, passport and NIE you go to the Doctors and sign up with them

Appointments are pretty hard to come by and there's always a big queue. We tend to use Helicopteros Sanitarias 24hr doctors instead but unfortunately the OH needs a regular prescription that can only be put on her medical card by a GP so we've just gone through the whole process a couple of weeks ago

Dawn, if you got your NIE from Fuengirola then you'll have the "newer" version which seems to be a form of residencia certificate too

Assume you've already got your social security certificate if you're paying it as I don't think you can pay your monthly fee without a number!
