Hi there
It is very useful to see the actual form and can see the context - haven't seen it before.
A bit relieved that it asks for last 5 years cancer only (Q21). However there is catch in Q17...

I assume a letter from GP should be sufficient as otherwise it would be hard to track the specialist. If specialist (NHS)...

Happy new year.

I had a stage 3 colon cancer in 2010 which was not spread and successfully removed surgically. I did not need chemo and am also in remission since then.

Can you I please check...
