Quick update: I mailed one of my 6166 original forms and the US 2002 Form to HMRC. While it was in transit my HMRC online account showed that something had arrived, presumably a 6166 from the IRS.

The TL;DR of this post is that HMRC told me on the 'phone today that they'd accept an original (not copied) Form 6166 and UK-US Individual 2002 directly from me, i.e. not via the IRS. More detail...

While this is a PLR, and from 2011, and so cannot be relied upon in the general case, it's pretty explicit that a rollover from one UK pension to another UK pension is not a taxable event for a US...
