We applied for PR through AINP in June 2013, Medicals were August, we paid the RPRF in November and landed in April this year. Hopefully won't be too much longer for you.

I put Canada for the ESTA when I was in this situation as I figured I was at least temporarily resident in Canada. (So it probably doesn't matter either way).

Put your country of birth as 'England' not UK and you'll be able to see it on Ecas

Winter sports insurance

I am here on a temporary work permit. Looking to buy travel/winter sports insurance. Will only be skiing in Canada. Grateful for suggestions, ideally a multiple trip plan.
