Yes, you will need to apply for a PC for both names.

Your fingerprints are not required for obtaining a UK PC but they will be taken on entry into the US.

We(hubby & 5yr old son) are currently in Twyford, Berks, been in the area for the past 12 years, I am originally from Zimbabwe. We were lucky enough to receive our green cards through the DV lotto....

Aah right, I only had to worry about my country of birth & residence which was 6 months for each, probably why I never noticed 12 months for all others. Are you resident in the UK?

We have just gone through the interview for DV2009, you need a PCC for every country you have lived in for 6 months or longer since you were 16. Good Luck!
