help needed for the health undertaking after arrival

Dear anyone,
please help me out. After I enter Australia I have to call the free toll number and they will set the appointment to appropriate health service authority.

I am from Bangladesh. Job verified in the mid of November. Probably they directly have a contact with our HR. They send to our HR all the docs I sent to DIAC and our HR posted a 3 page doc...

Dear MAhboob,
we wife had a medical history 10 years back. She had a tb scar but was treated properly. If you have some history (provided that it is not active) DIAC will ask you to sign this form....
