I picked up a nasty rash while in Provence 2 years ago. We ended up going to
the emergency room of a Nice hospital, where I saw a dermatologist. It cost
about 20 euros plus 5 euros for a cream. We didn't bother filing a claim
with our insurance company; the amount was so impressively low. (It turns
out there's some plant related to poison ivy in the French countryside.)

I have experienced something similar in France twice and in UK we have had free
NHS treatment although we are not in a reciprocal scheme. Kids under 16 get free
NHS treatment whatever their circumstances.

That depends on whether Italian hospitals are more expensive than the
inland tariffs or not. The insurance reimburses the standard tariff rates.
