Form SSA-1199
I want to switch my social security payments to another bank account. To do this I need to complete form SSA-1199. A section of this form has to be completed and signed by the bank. I have just contacted HSBC and have been told HSBC no longer carry out this service.
Has anybody encountered this problem and if so, found a way round it. Thank you. |

You will NOT need the bank to complete the bottom section; I didn't and the form was accepted when I went in person as I also provided 3-months of bank statements. |

I am in the UK and am aware of the correct form with the -UK suffix. Thank you. I live in Scotland and unless the change can be made at the US consulate in Edinburgh I am not going to bother. I can live with it. However the SSA-1199-UK form does require a signature from a bank employee. Did you have to get it signed by the bank? |

I know how you feel, I almost gave up as well and just stuck with my monthly exchange routine. A real "PITA". |
