URGENT INS Medical questions
Hello all -
Hope someone can respond to my post today or asap as I have my medical in a weeks time and am tearing my hair out about what to bring for my medical.
I am totally confused as to where the "1-693 Supplemental Form" comes from.
I have my I-693 - which was all I thought I should have. I do not have very many recent vaccination documents, so I suppose I'll have to pay through the nose for all that, too.
Does the civil surgeon supply, fill out and give me a copy of the I-693 Supplemental Form??? This form is apparently an Immunizations Documentation form.
It's just kinda alarming that I'd never heard of this form before today - and I'm a week away from the medical - arrrrghhhh!!!

I have never heard of it either,All I had was a signed letter from my GP,Saying what vaccinations I had.
I was given a copy of a form from the Medical but it says its a DS-3025.
Don't know if this is any help to you!!!

The Supplemental form is required if you did not have all the necessary vaccinations before your visa was granted. An INS approved physician will give you those vaccinations (if any) and will proviide the Supplemental form to which you refer.

is it true that you do NOT have to accept the vaccinations at that time, they just need to be done before adjustment of status?

sounds like the supplemental form IS the I-693. In other words the word
supplemental is the adjective.
