Packet 3 DS-230 - Questions

I've just received my DS-230 and I need assistance completing it.
I have a Family-based 2B petition which with priority date March 1994.
I'm already living in the States for 4 years with an H1-B visa. I
don't have a green card as of yet.
My questions are:
1) Just need to verify - all I have to send back is the completed
DS-230 part1, right? And that's it - I don't need to send any copies
of birth certificate, passport, medical documents, etc. Can somebody
clarify?
2) Since I'm already in the States with an H1-B visa - What should I
put in Questions #10 and #11 of the DS-230 form.
#10 "Permanent Address in the US where you intend to live if known
and the current person who lives there"
#11 "Address in the US where you want your Permanent Resident Card
maile"
Do I put my name and my current US address? Or do I put my petitioners
current address (not the same as mine)?
3) Question #25 "List Dates of all previous visits or residence in the
US". How do I fill this up? I've been living in the same address for 4
years, say a few vacations every year outside of the US. Under the
"From/To" fields, should I specify the dates of every exits and
entries from and to the US? or should I just say '02/1998-present' in
one single entry? (I hope this makes sense).
4) What is an "A" No.? (question #25 DS-230 part1) Where could I find
that? Considering my current status - should I have that?

You will send the DS 230 Part I, and you will also send the DS 2001, which
states that you possess all the documents on the list. No, the consular
post does not want you to send them the actual items on the list. I know
someone who sent them to the consular post, and the consular post
immediately sent them back.

Put your address in the the U.S. in #10, and "same as #10" in #11.

The latter.

You probably don't have an "A" no. yet. Just leave it blank. It will be
assigned to you, and will appear on your Permanent Resident Card.
