if any one can help me my fiancee received the packet 3 and 4 from the colombian
consulate in bogota and she told me that there is no form for the doctors examination
i thought that they send one of this to her i happen to have one here in the us
should i send this to her and have her take it just in case or do u think that the
doctors have those forms when she goes for the examination??????

i had my interview in cyprus, they too didn't send out a medical exam form... just a
list of doctors, i called one of the doctors and i told him that i don't have any
form, he said that's ok, i won't need it.

I don't think you need to send her that form. Generally those medical questionairs are passed out at the doctor's office where she will also have time to fill it out.
