Package 4 question

Hello, I received my package 4 but it doesn't have anything about the medical. I've
read on some sites that there should be a medical questionnaire included. I have my
interview the 30th so I'm very worried that I won't get it in time. Does anybody know
if the medical questionnaire for the US embassy in London is online, and if not does
anybody have the embassy phone number so I can call to request one.

The same thing happened to me. You have to call 0207 355 3335 (they close for lunch and at 4.30pm). They will send you details for the medical. Also make sure that you have been sent a payment slip too (They missed mine out) You have to pay £32 and take the slip with you.
