Need Help with 1-130 form

I need some help filling out the 1-130 form.
My wife is a US citizen and I am from the UK.
I am currently in the US on a B1 visa carrying out volunteer work, I am returning to the UK in May and so will not be applying to adjust my status.
1. Part C question 7. Asks for other names used. I legally changed my name in 1996, so I guess I would put that here. If so do I also need to send a copy of my change of name document?
2.Part C question 14. This asks to state if I am in the UK, how have i arrived.
Should I put on here a B1 visa or volunteer worker.
3.Part c question 15. This asks for the name of the employer. As I stated earlier, I am doing Volunteer work and not receiving any money. I am not sure whether I put the name of the organization I am volunteering for as my employer?
4.Question 22 asks. Complete the information below if your relative is in the United States and will apply for adjustment of status.
Your relative is in the United States and will apply for adjustment of status to that of a lawful permanent resident at the USCIS office in: If your relative is not eligible for adjustment of status, he or she will apply for a
visa abroad at the American consular post in:
As I am not going to be applying for adjustment of status do I fill this section in?
I realize that some of these questions may seem easy, however I really do not want to make a mistake and risk the application being thrown out.

No.
Not only is a certified copy not required (a simple photocopy will suffice), he's not being asked to demonstrate how or why he came to use a different name. The original should be brought to the interview... but nothing needs to be sent with the I-130.

If you haven't yet seen my 3 rules for filling out forms, here they are:
1) Take each question at face value;
2) Do not ascribe meaning to the question beyond what is plainly written; and
3) Do not try to alter the question to fit your situation.
By suggesting he include proof of his name change, you violate rule #2.
