Medical paper work, please HELP!
I have to hand in my paper work to Philadelphia, and when I went to
the INS website I noticed it said I had to have my medical done and
the papers ready to hand in with the rest. I'm getting all my paper
work from the INS webpage.
Today was the day that I went for my Medical. The thing was, is that
the forms I had were 4 forms off the INS page. One of the nurses
suggested I try and get the form needed from an INS office. So my
husband and I are thinking of going to Philadelphia and seeing if we
can just get the Medical Form for the doctor. Although the doctor
that I did see said that he would fill out all 4 forms for me. I
guess the paper work that is on an INS medical form normally is 4
papers but they're carbon copy. These forms are not that I handed in
when I went for my medical today, they were just 4 of the same thing,
which would need to be filled out 4 times, or photocopied?
Anyway my question is this, should I still see if I can get the
original form from INS and hand it in? Will just the paper work
printed out from the INS page still work? My blood work wont' be in
for a couple of weeks so the doctor won't be filling out any of the
paper work till all my tests are in. Can I just request a form by
mail by any chance? Anyone know? Any help would be appreciated.
