Dear All,

I'm now ready to send my K1 packet 3 to the London Embassy. Here is what I'm
including:

Short cover letter. Form IV-15 checklist (ticked and signed) Form OF-156 (completed
but NOT signed or dated) Form OF-156K supplement (completed but NOT signed or dated)
Form OF-230 Part 1 (completed, signed and dated) Attachment to OF-156 form (because I
needed more room to answer one of the questions)

My only questions:

1) Have I missed or erred on anything glaringly obvious?
2) Am I right in signing and dating everything EXCEPT the OF-156 and OF-156K forms?
3) I'm planning to just clip the forms together with a gator clip. Is this
acceptable?
4) I'm still worried that I might need the form OF-230 part 2 - sworn statement, but
the embassy didn't send it to me and the operators seemed unsure when I called
them (twice!). Does anybody here know?

Hi Joe. My fiance had his interview/got his visa today and they did NOT want the
OF-230 part 2. Didn't ask him a single question, either. Best of luck to you.
