Default IV-15 for K1 Visa Process

Hey People,

I have the IV-15 which I printed from the US Embassy London website at this link.

Is this the right one?
Also half of the document requirements don't apply to me like the Adoption Certificate and others. Do I have to write anything next to them or just leave the check box blank?

Hey, Ben.
Yes, it is the right one. I had the same thing as you. Half the docs didn't apply, so I just wrote N/A next to the checkbox, and the embassy haven't complained. I've already sent it with my forms, and they've opened a prov. file. Hi Katelyn!!

I was wondering a few months ago a lot of things like this and it helped me A LOT reading a web page from a couple (she is British and he is a USC)... it is very detailed and helpful.

If anyone wants it, I have the IV15 in PDF format for printing out.. Looks a
little more professional than the web page printed..
I also have FC-029 which im not sure is valid as its dated 12/98 but it is a
form you sign at the bottom stating along the lines that copies are exact
photocopies of originals which you may be required to submit if requested by
a Consulate officer .. I haven't seen it mentioned on any forms before but
If anyone wants either then please email me off NG directly at

Hey, Pete.
I remember seeing this FC-029 mentioned when I was researching. Rather than fill out yet another form, we just wrote the stuff about exact copies on our letters of intent, and our cover letter. We haven't had any problems, so, like you, I don't think the form is vital either. Yay!!! One less to worry about!!
