Default I-134 co-sponsor, same form?

I'm heading down to Havnana for my wife's K-3 interview in a ten days. I don't
think I should have any problem with the I-134 based on my salary which is well
above the requirement, but my current cash flow is low. Just to be 100%
covered I am planning to bring a form from a co-sponsor to give them if they
have any problems with mine. Is it the same I-134 or is there a different
form?
