Packet #4 is your interview letter...that came about a week after we called and got the actual interview date over the phone, which took about a week after he faxed over the correct checklist to Montreal. Did I just confuse all of you?

I don't see any following questions.
I can't answer the preceding ones because I have no idea whan you plan
to arrive and how long you intend to stay.

Q17 appesrs to aks if you have been in the US. This would be a yes or no
answer.

Some Embassies have their own version.
Remember, it is a State Dept form, not a BCIS one.
You should be able to do a search on DS-156 at state.gov and find it

Perhaps you couldn't find form DS-156 on the BCIS web site because it's not a BCIS form?

Try the Department of State website (DS - sound familiar?).

Why go to a commercial site for a form readily available from the source?
