Default filing-use my last name or my wifes maiden name?

When filing all the paperwork should my wife use my last name or
should she file under her maiden name?

Whatever name she wishes the documentation to be under.

your last name unless the question on the forms asked of her maiden name

There is no requirement in the US that the lady take her husband's surname
upon marriage.

There is no legal requirement for her to ever change her name, so why
would she not be able to make this choice for BCIS?
