FC-029 or FC-023 ?

This little form verifies that all photocopies are true and legible copies of
originals. I've seen it referred to by 2 different names: the FC-029 and FC-023. Is
there any difference? Does one supercede the other? Is it really necessary? The INS
doesn't refer to it specifically in any instructions I've received. By the way, I'm
doing DCF.

You do not have to have this form. You can make a simple written statement instead
and it works just fine.

However, If you are doing DCF, you are going to *have* to have originals to
show...make photocopies of the originals and show them the original and give them the
photocopy to keep. That way you get to keep your originals, which I recommend.

The FC-029 and FC-023 (both available from my website) are two sides of the same
piece of paper. You get them when you order forms. If you download, you don't get
them and must write your own "true and correct" statement.

The FC-029 is the statement that all copies are true and correct. The FC-023 is the
fee statement of all fees associated with the forms.

You can write your own.

You can write your own fee statement, but I don't think they would honor it. But you
could do it if you really wanted to. You could even include it with your check to
show why you are only sending $15.
