I got all the papers for AOS, and they're asking for form IRS9003 (additional questions to be completed by allapplicants for permanent residence in the United States). This document is not in my package and I couldn't find it on the INS site. Do I really need it?

Who is "they"? Your immigration practitioner? If so, he or she is behind

This form was required to be included in I-485 packages in the past, but it
has not been required since 09/21/2001.
