Affidivit of Support
Could somebody who has experience fill me in anything that I am missing or
has extra items?

Apologize for the incomplete post. I meant to save it then post it later.
Here's my continuation:
2) Photocopy Last year IRS tax return.
3) Current Bank Statement
4) Go to a bank for Notary.
Am I missing anything or anything extra and unneccessary?

Is this the I-134 or the I-864? And who is it for? A spouse getting
permanent resident status?

So, you said this is an I-134 affidavit for an F-1 student. Just a couple
of points: the I-134 instructions ask for a letter from the bank covering
certain information. Because this is for support of an F-1 student, I
suggest you get that letter and add it to the bank statement. The bank
statement does not include, for example, the total amount deposited for the
last year.
Also, the IRS tax return requirement is for self-employed persons. Is that
the case for you? If you are employed, you need to provide a letter of
emloyment covering certain information.
