OF-156 (London Embassy) - A few queries

Hi All,
I have (another) couple of queries whilst I put the finishing touches to my
application to send to the London Embassy.
First, I found out that the OF-156 they sent me when I requested the packet via phone
is out of date - the most recent one (dated 10/2000) I found on the official London
US Embassy website (www.usembassy.org.uk) seems to have more space to put things, so
I'm using that one. My query with regards to this is sending the printed-out version.
Should I cut the two sides to size (The OF-156 is a small form) and staple them back
to back or just leave them the full A-4 size?
Second, there is space at the bottom of the OF-156 for a photo to be stuck. At this
point, should I put the photo on it, or should I wait until the interview to give
them this (When I sign the form?) - on the latest version of the IV-15 checklist
they are now asking for THREE photos to bring to the interview, as opposed,
previously, to TWO.

You do not need to attach photo to the form-- I was told that when I called the
embassy and it didn'tcause any problem when i went for my interview. They told me
that all photos forms etc needed to be brought with me. I took 3 photos with me as
requested but they only asked for two---They may be in the middle of changing things
so I would be on the safe side and take 3 just in case I used the forms sent me in
the post so I don't know about using downloaded forms-- if you are in any doubt call
the embassy-- they are really helpful- I am afraid that I dont know the number but
I'm sure someone else will help on that( THere is a premium rate number on the
website and if you call that they will be albe to give you the direct line to the
visa section if no one here can supply it for you) Sarah
