Default childhood vaccinations

Hi ! I have been advised that I have to provide my employer with my vaccination booklet . Only thing is , I have absolutely no idea where it is

They want a record of all childhood vaccinations which would mean all those from the UK.

Does anyone know where could get such information ? The info is probably in the Kingston-Upon -Thames area dating from 1968.

And yes, I checked with my mother and she has no idea where it is either.
