Anybody work in insurance?

Had something a bit odd crop up today.
I was getting insurance sorted for my new wheels and towards the end of the hour and twenty minutes I spent on the phone, the woman on the other end pointed out that I'd had a claim in 2011 that I hadn't mentioned on any forms.
"Ermmm... I haven't ever claimed," said I, truthfully.
"We're showing a no-fault claim and pay-out with such and such an insurance company* in 2011," said the woman on the other end of the phone.
So apparently they have a claim showing on my file from nearly four years ago, involving a 1999 vehicle.
This is not right. I've never owned or driven a 1999 vehicle and I've never made an insurance claim, least of all in 2011.
The insurers couldn't give me any other information (I think the woman was new) but if anybody works in insurance on here and can give me a pointer on how to find out more, I would be grateful, because to all intents and purposes it now appears as though I lied on my insurance application.

I don't work insurance, but I am fairly sure you have a right to see your file. Phone your current insurer back and ask their procedure for you to obtain a copy of your file, and make sure you state that you believe it contains significant errors in it. You will probably need to write in to request the information. Indeed, your documentation may already contain a contact department to whom you can make the request. Best to sort it out fast as an ongoing error like that could mean your insurance can be shown to be invalid were you to make a significant claim.
