Default 1042-S on US investments

Just wondering if any of you have US equities, ETFs, etc. for which you receive dividends in your investment account or TFSA and whether you got (or expect) a 1042-S covering the 15% US withholding. (And the account is held with a Canadian financial institution).

The IRS is telling me I absolutely should have gotten one and the people I deal with are saying they absolutely aren't going to issue me one.
