Where to send e457 med forms?

Where to send e457 med forms?
Hi, We have applied for e457, had meds (unrequested), completed the printed form manually & gave it to DR, but were not sure which address they should forward the completed med form to. We just put our application code and wrote 'on line' in the address box. Do you think this is ok, if not how do we find out the address to send to for an e457 med form to be sent manually? I said I would let the dr's receptionist know. She is waiting for blood test results anyway before sending the form on. Many thanks, FlossandJO

I am affraid you cannot sent directly the med results/forms.
It has to be sent through the panel doctor directly to the LCU in Sydney.

Thanks for your reply. I know we cannot send the form ourselves. It was the address that the dr is to send the form to that I was checking. Am I correct in thinking that Sydney processes all e457 applications?
