When Do You Need To Arrive

A question or two that may have been asked before and apologies in advance in case they have.
Wife will be 45 in August 2008.
She is a nurse (21yrs) and is looking to continue in her profession.
If she applies for a visa does she need to set foot on Australian soil with it in her hand before she is 45 or just be granted it before then?
We have a son in Uni and a parent with Alzheimers who we jointly look after. Whilst I think that the son isn't a problem (he doesn't present himself as one) the question of how far along the path to oblivion will the parent with Alzheimers be by the time she is 45 is an unknown quantity.
I know that I may sound harsh but when the time comes that I'm not recognised is the time to say "I don't need to be here anymore" (for my own sanity more than anything and it's my dad!) and leave it to the health professionals and get on with making a new life for me and my family.
My instinct is to start the ball rolling, pay the fees and accept the risk of it all coming to nothing. At least if things are in our favour by next year we will be in a position to act immediately.
Also, our son would be 17, what are the chances of him being able to land in Oz and apply immediately for a course in the construction industry.

Regarding your wife's age - she has to be under 45 at the date your application arrives with DIAC.
