What Address do I ask for our Meds to be sent to in Sydney ?

What Address do I ask for our Meds to be sent to in Sydney ?
Hi.
We have booked our chest xray for tomorrow and the clinic has told us we must bring the form downloadable from the Australia House website.

How ever I see on the form instructions it says to the doctor to forward the xray to the address specified on the Covernote or "Office Use Only" section of the form.
We haven't had a CO hence no official request or cover note for the Meds yet....can anyone tell me what address (in Sydney?) we should ask the doctors to forward the forms to...presumably with our acknowlegdement reference number ??
Any help appreciated as we don't want to muck up and loose the forms in the system at this stage
(we've expecting a CO in the next few weeks but want to minimise any delays)

When we went for our meds the receptionist asked us if ours were going to the LCU in Adelaide or Sydney.
Our were Adelaide.
So it may be an idea to e-mail and ask for confirmation of where to send them
