If you are in the UK, you have to use the specific form from the specific Police Force - for example, if you are in West Yorkshire, you must you the West Yorkshire Constabulary ones - not the MET Police ones. Just go to your local police station & they will supply them to you.

yep thats what we did, went to the local police station and the forms were sent to us within 3 days
