for the initial medicals we didnt need to request GP information, just filled in the history questionnairre, also took specialist report for OH diabetic bi-annual check ups. However we have been now asked for a major specialist report by LCU Adelaide.
You might not need this - unless you are needing specific information about your history - dates etc. We just tried to remember our age at time of operations, wether it was spring, autum etc to get month and year.

Our Panel doctors letter stated that if there was any medical condition that we should bring the reports relating to the condition from GP along. But when I spoke to them, they did stress to ask for a photocopy thinking that it would be cheaper on the cost for us. They were partly correct, the GP charged us about £2.16 for the copies but £10.00 labour charge for a 7 page report print out.
Maybe check why they need it from the Panel doctors before you go ahead and pay it.
For us, our panel doctor has requested all the info upfront as he feels that it may save us time in the long run rather than waiting for LCU come back later and asking for it which we agree to.
Let us know how you get along

My husband had a major operation 19 years ago which has left some scarring on his lung. He made an appointment with our GP, explained the situation to him and she hooked out the relevant documents there and then.
She then photocopied them and handed them over free. She said that h has a legal right to access to his records so there was no way she could keep it from him.
I then hand delivered the docs to the panel doctor who has now sent the whole file of to LCU.
