Medical/ X-Ray Forms
Dear all
We are planning to undergo unrequested medical test tomorrow, just wanted to confirm whether we need to send these 2 forms only:
- For X-Ray, Form 160
- For Medicals, Form 26
And this needs to be sent to ASPC Adeliade....correct?

The forms are correct though - dont forget your passport as they check them against the piccies on the forms.

These medicals must be sent to the Local Clearance Unit in Sydney.
•If medicals are sent via normal post, the address is –

Just to confirm, I had sent my Immg appl to ASPC Adelaide, whether they will able to trace to LCU?

When you complete the forms, make sure that you put your case file number on them (in the box headed "For Office Use Only"). That's how they match up your medicals with the rets of your application.

We have sent our Meds and X-Ray directly to DIMIA by DHL. That was a huge envelope. They made X-Ray 60x60cm...

You need to be sure to put your ref/case number on the forms/xrays so they can be tied up with your application. You should have had this ref number back with your acknowledge of receipt from DIMIA I think.

So, they made all the tests, sealed the results in their official envelope, with all the stamps around etc, and I sent it.
I was afraid that usual mail is not safe enough.
