We have recieved medical forms for me and my spose. Due to pregnancy our case was on hold and haven't recieved forms for the baby. I got the medical forms from an agent , but doesn't have baby's photo stapled on it along with the DIMIA stamp. Can i use the same ? The forms that i receieved from dimia has thoer stamp on the photo stapled.

Have you asked your agent about this? If they couldn't answer I would probabaly get them completed and a photo of the baby certified and sent with form to adelaide as the surgery send them direct in order to avoid delaying things.

Yes, or solicitor. I got some of my documents certified by a GP so maybe when you visit for your medicals he/she may be able to do this for you on the spot if they are feeling kind.
