Hi friends,
I read in paper lodging check list some document like form 1276,Australian value statements and form 80 should be filled and signed.
If i will lodge my file online should i download these document and fill it then scan it or in online application these forms will be filled electronically.

Fill online application, and then check the 'Document checklist' in online status tracking page. Whatever form is mentioned thr (a link to the form will also be given normally), just fill it up and upload. For instance, in my case I only have 1221 to be uploaded in my doc check list.

Apart from tht, u may want to fill up form 80 and keep it ready. Or you may front load it too even if not asked in the doc checklist. No harm.

I did an online application but did not fill form 80. My CO requested it before my grant
usually the details in form 80 are such that they are pertinent at the the time of grant... but to be on the safe side , there is no harm filling and sending it to the address given for documents.

I have duly filled both the forms and submitted online...apply fast as MODL are going to revise...

I applied on 11th aug for GSM 175 CSL visa, it didn't ask me to fill in form 80 or 1276, and in my document checklist it does not appear....is this right as after reading the above i am worried that i should have done this.

Yes, i have asked by my agent to fill both the forms...

Hmmmm i'm puzzled now as they were not part of the documents requested by the online application, i'm not using an agent and doing this myself and of course i don't have a CO yet so no-one to really ask

Is 175 checlist pdf on immi's website applicable for online applications as well?

From my experience the checklist is completely different as it doesn't ask for any of the forms mentioned in this thread, the only documents its requesting is the medicals that is all plus all the usual things like birth certs, passports, evidence of work experience etc.......hence i am confused!!!

It is perfectly alright. Form requirements may vary across applicants based on their circumstances and what they have answered to various questions asked during online application filling process. Even I am also not asked the forms mentioned above, and I have not uploaded them either. Probably wud upload the form 80 though, as more often than not apparently the CO would ask it at a later stage.

By the way, haven't you been asked to upload even form 1221? It is there in my checklist.

Hi, not i have not got form 1221 on my list as being requested...as i said before its only the medical forms and radiology forms i have been asked to upload....strange it varies so much from person to person
