Health Undertaking
In our letter that came with the medical and police check request there is one health undertaking form. We have filled this out for the main applicant (my husband). Do we need a form for each person on the application? If so how do we obtain an extra form. I have checked the forms on www.dimia.gov.au and can't see one.
Advice would be appreciated

Just one for the main applicant just fill in top left adn sign and date and return.

We are front loading our med apps, do we need to include this form?

If one is frontloadong medicals, where can one get the Health Undertaking Form 815? It is not there on the immi website.

DIMIA/ASPC usually sends it with the request for medicals. Mine came as a photocopy of my case officer's original (in fact, most of the forms she sends me are photocopies except for the medical forms).

The health undertaking form is not a mandatory form to include. If you have no health issues at all, you won't need to give an undertaking.
