Default Form 160 - How to get an additional one for my 11 year old son?

Dear Friends,

My son turned eleven in Sep 04 and I received the "yellow envelope" on
3 Jan 05 which only contains two forms 160 i.e. one for my wife and one
for myself only. How can I get another form 160 for my son which is
missing? My deadline is to submit everything by 4th Feb 05 (delayed in
post) and in that situation I am doubtful if I have time to apply and
get a new form from the CO unless that is the only way to get it.

Please advise if there is any alternative way to get the forms faster.

Try emailing the Australian High Commission in London. Or better still - fax them.
http://www.australia.org.uk or look at the DIMIA website for contact details.

At the same time, email your CO, and ask for a new form. They will give extensions to deadlines if there is a reasonable basis for doing so.

You can send an email to [email protected]

Include your name, the number of people on your application, a list of who is on the application, and their ages, your Postal address, and the visa type you are applying for.

I emailed them on Thursday and received the forms today (saturday) from the London embassy.

Dear Rubbeldiekatz,

I tried to download per your advise but noticed that none of the 3
forms are useful, form160a for use in OZ only, 160Mp is for Medical
Practitioners only and 160ch is Chinese version.

Strangely, could not see just form 160..

I will try to e-mail to "[email protected]" and wait... But will
welcome any additional ideas in the meantime.

Sam try not to stress.

It happened to us!

Email you CO, he/she will send one straight away and give you an extention.

Also meeting the deadline for return of medicals was/is going to be tight. CO happy to extend the deadline when we told her the date of our meds.

I am in the same boat, I booked meds at a local clinic for the 31st Jan, my son turns 11 on the 4th Feb but I have no form for him, I asked the clinic if they had spare forms and they said "YES" dont worry.

Try calling your clinic and asking !!

confirmed having an extra form... Will go there thsi Friday and
complete.
