Default E Health forms

daft question perhaps but can someone who has been through the process please advise...

doing the e457 and the message window has medical forms to be completed link. Bit confused...do I do these now? If so, what do they actually do at the medical? ( I mean in terms of paperwork)

Hiya,

Don't quote me on this, but i think that's the bit the doctor completes. we had our meds today, quite straight forward, nothing too complex or to worry about.

On you on eHealth or snail mail?

Have you already completed a health questionaire, & printed off xray & med forms?

Our doc said that he would do his bit on friday, as long as he had received all of the results back. (we had to have blood tests), so will know when its completed if the message changes.

Sorry dunno if this helps or creates more questions!

no haven't printed off any forms. Just assumed this would be done at the medical. They didn't say anything to me when I booked the appointment and there's no mention of any forms in the confirmation paperwork they sent. That's what is confusing me!!

We opted for eHealth, in which we completed the health declarations online, then printed off an "Examination Referral Letter" which details what you have to have done at the medical. (ie the medical itself, xray, blood tests). We did have to take these letters with us, although the doctor took one quick look and did nothing with it, but the xray dept photocopied it. Also check with the doctor whether you need to get you medical history files from your GP, apparently this can cost a few quid depending on your GP. Fortunately they didn't need them for our medical.

Sorry missed at bit from your post. If they didn't mention bring these referral letters in your confirmation details, did you opt for "ehealth"? Did you complete the health declarations online? If not it sounds like you didn't go the ehealth route.

I think you fill in the forms prior to your appointment, there is a section for each applicant to fill in about medical history, take these forms to the doctor where there is a section of the form for the doctor to fill in. This is what we did. You will also need to take passport photos of each applicant and passports for ID.
As for what do they do at the medical...not a lot, listen to chest, look in eyes, ears, feel abdomen, blood pressure that was about it
If you need xrays there are separate forms to take with you and you will need more photos for these too.

Just thought there are only a handful of doctors who operate the electronic health system if your chosen panel doctor doesn't do ehealth then you download the manual forms and complete them as described above....this is how we did it but the ehealth option may be different
