could anyone tell me where i send a change of circumstances form.

im leaving for oz in 2 weeks does anyone know about the whole change of circumstances, circumstances. a mean will my situation have to sorted out before i go? also passenger card is that just my plane ticket?

Depends what your situation is as to whether it needs sorting before you go.
As for the passenger card, everyone has to fill one in before landing. You may be given one when you check in at the airport, if not you will receive one whilst on the plane.
