Requesting CAIPS notes form US
I have a friend in Canada that is going to request CAIPS notes for me. What is the address he needs to send it to, and do I need to write an authorization letter too?

Yes, you have to send an authorization letter which can be downloaded from the website.

I am not sure if an authorization letter on a plain paper would be enough, if you cannot find an authorization form from the website. As I live in Canada, never bothered to search for the authorization form.
