Default Requesting CAIPS notes form US

I have a friend in Canada that is going to request CAIPS notes for me. What is the address he needs to send it to, and do I need to write an authorization letter too?

Access to Information and Privacy Coordinator
Narono Building
360 Laurier Avenue West, 10th Floor
Ottawa, Ontario K1A 1L1

Yes, you have to send an authorization letter which can be downloaded from the website.

The caips application form can be downloaded from http://www.cic.gc.ca/english/pdf/kit...s/IMM5563B.pdf.

I am not sure if an authorization letter on a plain paper would be enough, if you cannot find an authorization form from the website. As I live in Canada, never bothered to search for the authorization form.
