Question About Filling out name in Native Script

I need some help on filling out the form for independant immigrant. This is regarding
Native Language.
1)I am from India and my Native Language is not English. In the form, it asks for
filling out the name in Native Script. Is this applicable to people from India? (I
cannot write or read my Native language, but can do so in another Indian
langauage). All my records are in English

2)My wife and I have have different Native languages. We have a new baby now. What is
her Native language? She was born in the USA, but has not yet started talking. (I
have given some background below which might help).
Any enlightened souls out there? Please help!

Background: My wife can read/write in her native language. Are 'mother tongue' and
Native language the same? I assumed so, but the dictionary and other sources I
referred to give different interpretations! Is it normal that mother tongue is
father's native language, since most of the people are from Patrilineal backgrounds?
Very confusing, but maybe there is a simple answer. BTW, we speak a third language at
home, and that is the most commonly used language at home. This is the language that
both of us can speak fluently. We try to talk to the baby mostly in English because
the pediatrician didn't want the baby to get confused because we switch langauges
depending on who we talk to!!
