"Did you co-sign an undertaking where the sponsored family members received social
assistance/welfare during the validity of the undertaking?" Yes/No

Should I ignore this question in "F - Eligibility Assessment part 2"

Also there are some sections like

D - Adoption Case G - Residency Declaration For Canadian Citizens Living Exclusively
outside of Canada H - Consent

on this form. Should I leave it blank since I that don't apply to me! If I should
write "N/A", which field I should use?

There is something like "For xxx Only", there mean I can leave it blank if that is
not for me?

Thank you for your help!

These are simply questions to determine your eligibility as a sponsor or cosignor. If
they do not apply simply indicate no. If these speciality forms for adoption and
Canadians residing outide of Canada do not apply and you are not providing consent
for the release of information on your case simply ignore them. Ron Beirnes

IF THIS IS THE FIRST TIME THAT YOU SPONSOR A FAMILY CLASS RELATIVE, YOU CAN SAY TO THE QUESTION.

IT IS A GOOD IDEA TO ANSWER EVERY QUESTION DESIGNATED IN THE FORM.

>on this form. Should I leave it blank since I that don't apply to me! If I should
>write "N/A", which field I should use?

YOU MUST KNOW THAT N/A MEANS NOT APPLICABLE. IF THERE IS A BLANK TO BE LEFT, YOU MAY PUT N/A THERE. I RECOMMEND TO PUT N/A IN EVERY BLANK YOU WILL LEAVE WITH THE FORM.

>There is something like "For xxx Only", there mean I can leave it blank if that is
>not for me?

YOU ARE RIGHT. YOU MAY AS WELL PUT N/A THERE.
