Actually the enclosed letter does not mentioned DMP but it was
addressed to my physician (and i assume to go to my regular family
doctor)..since our origional medical was done 3 months back and DMP
sent the report to canada office and in reply to that they want me to
do some additional tests....

Hey nut case,
is it so difficult? the letter is addresed to your physician. So ive the goddam thing to him. It will have all the instruction on what to do.
