Default no medical forms enclosed

Greetings,

In April 2002 I received a notification that my file has been transfered
from Buffalo to New York and that and interview will be scheduled.
A few days ago I received a notification from NY that the interview has been
waived and that I have to proceed with medicals. The problem is
that no medical forms/information has been enclosed in the envelope.

Is this a mistake from their part or do I have to get the forms online
somewhere? Do I have to send a fax to the Consulate in NY requesting
the forms?

Its a mistake if they asked you to take the medical exam but did not supply
any forms or other instructions. Write and explain what happened.
