Hi, I am single and submitting the independent immigration application only for myself. I
wonder if I should or should not include the form IMM5406 (additional family information).
If I should, then, how many copies will be required? Please let me know. Thanks!

Hi:

You must fill out an Additional Family Information Form (5406). You must provide 3 copies
of this form.

That depends on the office to which you are submitting the case. In most cases, the
IMM-5406 is required only if an applicant has been married more than once.

David,

I apologize for correction but IMM 5406 has nothing to do with the number of marriages.
Most visa posts require IMM 5406 to be includes as it contains information about parents
and siblings which is not included in main application. Some visa posts (i.e. London) use
their own variant of Additional Family Information form.

Ingrid, David, and Andrew, Thank you for your replies. I am sending the application to
Buffalo, and actually, I have to include family census register from my own country (not
USA), so that shows all related family information. However, based on your comments, I
will also include IMM 5406 to be safe.
