Help - Wrong Address Medical Forms

Default Help - Wrong Address Medical Forms

I just received my initial assessment -waiver- with medical forms. I would
really appreciate is someone can help me with a couple of questions:

1) The medical forms have my previous address even though they were mailed
to the correct address. Should I do my medicals and notify Buffalo? or
Should I wait until I get new forms?

2) I am Mexican and we go under two last names (father & mother's last
name). So my passport shows xxxxxx yyyyyyy as surname, and a single name
under first name. Now, when I received my medicals, both my wife and mine,
have our first last name xxxxxxx as the surname; and in the first name box,
we have our second last name yyyyyyyy and then our name. Is this regular
practice? Or should I contact Buffalo and let them know about it?

Buffalo 10/09/01, AOR 11/16/01, Assessment 10/17/02.

there is no need to inform of the address.Do the medicals and send the results to the consulate that is enough.It will be the old address on the medical forms. Best of luck.

Regarding address on the medical forms, don't worry if it's not the same. I received exactly like you the assessment papers on the new address, even on the medical forms and assessment letter it was the old one. The doctor will correct it if (s)he thinks is necessary.
In the second matter I suggest to contact them immediately because you may run into problems, if not now, later for sure. Me and my wife also have two names in surname box, but they kept everything exactly as on the passports.

Do the medicals, remind the visa office of your new address, and do not
worry about the names situation.

Thank you for your help and prompt answers. I'll just write a letter along
with the additional papers that they asked for. I guess it will not hurt to
point it out, but I'm sure they know a whole lot more than I do.
